This came from one of two 12" (not 10" as I stated in the original description) blue-label Columbia Masterworks records, catalog #10001-M ("Noise Spectrums") and #10002-M ("Warbled Frequency Record"), from the label's Technical Series, matrix numbers CT-4-1 and CT-5-1. The unlabeled back side of each record was just a widely-spaced blank groove.
