Just your typical scream of pure evil from the depths of Hell. Mix it in far into the background of a scary movie to give the audience a subtle feeling of unease, or at full volume for a truly horrific effect.

  @darkflamesquirrel: you are pretty close. The scream is time-stretched to be about 15% the original speed. Then there is a formant shifter(-6 semitones), followed by a liberal amount of reverb, and finally some heavy distortion. It's crazy how you can turn some goofy-sounding shrieks into horrifying screams with just a few effects. This was inspired by the distorted scream in Aphex Twin's "Come To Daddy."
